YOU'RE NOT ALONE
You’re probably wondering what will happen when you leave the children’s hospital. You might be asking ‘who is going to look after me?’, ‘will my new doctors know what to do?’, or ‘how will I cope having to start all over again with a new team of health professionals?’.
You’re not alone – most young people who have chronic conditions ask these questions. And many more!
Trapeze is here to support young people with chronic conditions. The government has funded our service so we can walk with you on this journey.
MAKING YOUR TRANSITION A POSITIVE EXPERIENCE
Trapeze is here to make your transition a positive experience, as best we can. From our experience, and what we know from young people, transition can be made easier if you start early and plan well. Once you turn 14 you can call us and we can help prepare you for the big move. You may think 14 sounds too young, but this will allow us to get to know you better and understand what you need in your transition.
